AlzWell
Benevolent Hearts Home Care recognized that today’s current model of dementia care in the home lacks caregiver training, education for the families and structure that is conducive to the current level of the disease. Due to the frequent changes in behavior, a plan of care must be monitored regularly as the disease progresses. That is why we created AlzWell, a specialized program for persons living with dementia and for the families who care for them.
There are many forms of dementia including Alzheimer’s, Lewy body, Fronto-temporal, Vascular and others. Dementia is a progressive disease and there is no cure, but we can help care for your loved one who suffers from dementia or other types of brain change.
AlzWell, provides a person-centered approach delivered by a caregiver that is trained by an Independent Certified PAC Trainer and will create actives and encourages music therapy. Our Care Managers conduct frequent case conference and provide resources for the family.

*In accordance to the guidelines provided by Teepa Snows Positive Approach to Care (PAC), we now have access to PAC Certified Independent Trainers on Consultants to guide learning and skills as supported by his or her certification type.
- We conduct an in-depth assessment in your home by an AlzWell care specialist and activities will be developed specifically to match the clients’ interests and changing abilities.
- Our trained caregivers demonstrate supportive techniques used in PAC training including music therapy.
- Case conference will be conducted by a Care Manager along with the family on a frequency of every week for clients we serve 40 hours or more. Less hours will result in bi-weekly case conference.
- The family will receive the “Dementia Care Guide” written by Teepa Snow and be given community resources for support in caring for their loved one living with dementia.
Benevolent Hearts wants to give back and to honor that commitment, our Independent PAC Trainers voluntarily provide training to senior care facilities in our community. We also partner with the North Central Alzheimer’s Association in raising awareness and to help fund in the research for ending Alzheimer’s. Benevolent Hearts Home Care is a partner of Dementia Friendly Fort Worth.

Who is Teepa Snow and what is PAC?
Teepa Snow is one of the world’s leading advocates and educators for anyone living with dementia or other forms of brain change.
Teepa is an Occupational Therapist with over forty years of rich and varied clinical and academic experience. Her philosophy is reflective of her education, work experience, medical research, and first-hand caregiving experiences. Her advocacy efforts led her to the development of the GEMS® State Model for understanding the progression of dementia and changes in abilities. She also created the Positive Approach to Care® training strategies, which are effective techniques for anyone seeking to optimize care and support for those living with brain change. As the Education Director of Eastern North Carolina’s Alzheimer’s Association, she also helped to create the nation’s award-winning DVD entitled Accepting the Challenge: Providing the Best Care for People with Dementia. Her user-friendly approaches provide guidance and leadership to national efforts to promote best practices in care.
